How to use it

  1. Put your privacy notice next to the sheet (who you are, why you collect the data, how long you keep it, how to withdraw consent).
  2. Ask the guest to tick yes or no for each purpose separately: never pre-fill the answers.
  3. Collect only what you use: an email or a phone number is usually enough.
  4. Store the sheets locked away, copy the contacts only for the purposes marked yes, and destroy them when no longer needed.

Frequently asked questions

Do I need consent to send offers to restaurant guests?

In the EU, marketing emails and SMS generally need prior, specific consent that you can prove. Booking reminders that are part of a reservation are usually a different legal basis. Check with your local data protection authority.

Can I use one tick box for everything?

No: consent under the GDPR must be specific, so each purpose needs its own yes/no. Bundled or pre-ticked consent is not valid.

Is this form legal advice?
